Hospice care focuses on the care, comfort, and quality of life of a person with a serious illness who is approaching the end of life. At some point, it may not be possible to cure a serious illness, or a patient may choose not to undergo certain treatments. Hospice is designed for this situation.
“As a LMSW with more than 20 years of hospice patient care experience, I know how important it is for families and their loved ones to receive professional, compassionate, and dependable services during a difficult time. I’ve worked with this team for more than 5 years, and know first hand how they take care of families as if they were their own; focusing on care coordination with the hospice team and caregivers, and also the little details that are above and beyond traditional care which makes all the difference. Often families are not sure where to turn during this difficult chapter, they are, understandably, overwhelmed. Solutions for Seniors brings families comprehensive care, coordination with other available community resources and ultimately peace of mind.”

Geriatric Care Managers (GCM) are typically licensed nurses or social workers who are trained in senior care and act as private advocates for seniors. GCMs are specially trained to help develop an effective care plan by assessing the current situation and identifying resources to meet the needs of the senior. Senior Care Solutions and Services is one of those resources that can ensure the senior’s needs are being met and help oversee the individual's care plan.
“I have worked with this team since 2015, when my wife, a certified Aging Life Care Professional, and I started AGB and Associates Geriatric Care Management in Pawleys Island, SC. They often partnered with us when their clients required a higher level of care management than the care managers were able to provide. Similarly, we often recommended this team to our clients who needed in-home care. During the seven years we have worked together, I have observed them provide home care at the highest level of professionalism, including management of multi-disciplinary teams consisting of nurses, therapists, caregivers and care managers. During that time, we have collaborated on a number of highly complex cases requiring 24-hour caregivers and coordination of a variety of health care providers, including physicians, physical therapists, and mental health professionals. They have consistently upheld the highest standards of the profession.”

Physicians diagnose and treat injuries or illnesses and address health maintenance. Physicians examine patients; take medical histories; prescribe medications; and order, perform, and interpret diagnostic tests. They may counsel patients on diet, hygiene, and preventive healthcare.
Through their work in the senior care industry, Senior Care Solutions and Services has provided excellent care and support for many of my patients. They have been a stalwart advocate for their clients and numerous times exceeded the expectations and standard of care for our vulnerable population. They’re extremely effective in coordinating and facilitating clients’ needs—from creating solutions-based care plans to providing and implementing referrals for legal and fiduciary assistance.”

When families don’t know where to turn, Senior Care Solutions and Services for Pawleys Island has the answers, expertise, community partners to collaborate with, and commitment to find the best outcome. Our team followed Mr. Boyd from Arbor Landing in Pawleys Island to Encompass at Waccamaw Hospital to Georgetown Hospital to Kempton of Charleston and finally to the Veterans Village in Florence.
So very, very, very proud of our work for this family and the collaboration/teamwork between all these partners to help support this patient and his wife. Focusing on finding best patient outcomes, not just placement, and seeking solutions for success is all part of our mission. Our team pulled together the plan, managed the coordination, all the communication, and direction with each provider. They met with the nursing and rehab team in person. We were on site as the patient arrived, had him settled and set up for success!
